The Electronic Asthma Management System (eAMS)

Description: The eAMS engages patients with asthma to complete a pre-visit questionnaire through a web portal or app and processes this information to give providers guideline-based advice to optimize therapy, personalized to each individual patient, through easy-to-use prompts built into their electronic medical record system. The eAMS also creates a self-management asthma action plan, which patients can access through their app or web portal anytime, anywhere. This empowers patients to self-manage their asthma, improving their symptoms and helping them to avoid asthma flare-ups and urgent healthcare visits.

Quality Standards Summary for Asthma in Children and Adolescents

Description: This tool synthesizes content from the Asthma Care in the Community for People Under 16 Years of Age quality standard from Ontario Health. It is intended for use by health care providers and caregivers. It covers all six of the quality statements, including: confirming a diagnosis of asthma in children and adolescents, asthma management; referral to specialized pediatric asthma care, and follow-up after discharge.

Provider Asthma Assessment Form (PAAF)

Description: The Provider Asthma Assessment Form (PAAF) is a point of care asthma management tool for primary care EMRs. It is adapted from the LHF asthma care map and includes the PRESTINE asthma data elements, asthma performance indicators, and an algorithm to identify severe uncontrolled asthma. To date, it is implemented in OSCAR at the Queen’s Family Health Team.

Pediatric Asthma Action Plan

Description: This tool outlines steps for patients and their caregivers to self-manage asthma. Topics include ‘how to get the most out of your visit with your healthcare provider’ and developing a pediatric asthma action plan.

My Asthma Diary

Description: My Asthma Diary is a helpful self-management tool developed by Lung Sask. Patients can use the asthma diary to record their information, symptoms, medication use, peak flow meter readings, and asthma triggers.

Mild Asthma Decision Aid (Patients and Care Providers)

Description: The Mild Asthma Decision Aid is a tool designed to engage patients with asthma and their care providers in discussions about current asthma treatment options. The tool helps them to reach a shared decision on treatment that is based on scientific evidence and each individual patient’s goals and preferences.

Description: This suite of resources was produced by Lung Sask to provide information and education on inhaler use for patients, across a range of devices.

How to Properly Use an Inhaler

Description: This online tool provides a collection of videos that teach users how to use inhalers/puffers properly. Videos are available in English and French and are developed for adults and children.

GINA Patient Guide: You Can Control Your Asthma

Description: The Global Initiative for Asthma Patient Guide is an information guide for asthma patients and their families. It focuses on achieving asthma control and I available in English, Chinese, German, French, Spanish, Swedish, and Turkish.
